Attendee understands that participation in the live portion of the SNIS 19th Annual Meeting & Fellows Course, July 25-29, 2022 (the “Meeting”) is entirely voluntary and requires attendees to abide by any applicable rules of conduct or local or state laws that may be announced at any time during the Meeting. In consideration of the right to attend the Meeting, Attendee assumes all risks and accepts sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that attendee may experience or incur in connection with attending the Meeting. While the Society of NeuroInterventional Surgery (“SNIS”) is the convener of the meeting, Attendee understands and acknowledge that SNIS has no direct control over and is not responsible for the acts or omissions of any meeting venue personnel or others involved in producing this event, or any other attendees at the meeting. Attendee has carefully considered the risk involved and hereby releases, covenants not to sue, discharges, and holds harmless SNIS, its officers, directors, employees, agents, and representatives, of and from any such claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind (including reasonable attorney’s fees) arising out of or relating thereto.
Specifically relating to the global COVID-19 pandemic, attendee acknowledges the highly contagious nature of COVID-19 and voluntarily assumes the risk of exposure or infection by attending the Meeting, and understands that such exposure or infection may result in personal injury, illness, disability, and/or death to attendee. Attendee understands that the risk of becoming exposed to or infected by COVID-19 at the Meeting may result from the actions, omissions, or negligence of others who may attend the Meeting or their families, colleagues, or others with whom they may have contact. Accordingly, attendee understands and agrees that this release includes any claims based on the actions, omissions, or negligence of SNIS, its officers, directors, employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in the Meeting.
In addition to all other rules and regulations relating to the attendee’s attendance, attendee agrees to comply with all COVID-related procedures that may be implemented by the venue facility, including, but not limited to, mask-wearing and social distancing requirements and restrictions on certain activities that carry higher COVID-related risk, in order to protect as much as possible the health and safety of all SNIS meeting attendees.
By completion and submission of the meeting registration form, I certify that I have read and fully understand this Liability Waiver and Assumption of Risk agreement and understand that it affects my legal rights. I agree and acknowledge that this waiver and release (1) shall be binding on me, my heirs, family, estate, representatives, and assigns and (2) is intended to be as broad as permitted under applicable law, and each waiver or provision herein is severable and shall apply notwithstanding the invalidation of any other provision herein.
